Order Live:On Kits

Patients

As a newly diagnosed patient with a short time period to bank sperm, you should order the Live:On kit directly by calling (800) 466-2796. The order system below is for healthcare professionals only. The cost to order a kit is $65. When the kit is returned to Cryogenic Laboratories, Inc. (CLI), you will be billed $610 for processing, analysis, freezing, and one year of storage*. The total cost is $675.

The cost for additional kits shipped back to CLI within 30 days is $355. This price also includes the kit, shipping, processing, analysis, freezing and one year of storage.

Healthcare Professionals

The Live:On kit is provided free of charge to healthcare professionals for distribution to patients. When a patient returns the kit to Cryogenic Laboratories, Inc. (CLI) they will be charged a total of $675. This fee includes the following:
  • $65 for the kit and shipping,
  • $290 processing, analysis, and freezing
  • $320 storage for one year.*
The cost for additional kits shipped back to CLI within 30 days is $355. This price also includes the kit, shipping, processing, analysis, freezing and one year of storage.

* Monthly storage plans and discount longer term storage plans are also available for the client’s selection.

Please note the kit has a 6-9 month shelf life and needs to be refrigerated until distributed to patients. We recommend that you do not order more than you will need during this timeframe.
